Going home after a stay in hospital can be very daunting especially if you live alone. This service gives support to older people on discharge from hospitals in the local area - the RUH, St. Martin's, Mineral Water and Paulton.
We take the patient home and help them to settle back into their familiar surroundings making sure they are safe, warm and comfortable.
Short term help is given, if needed, which can involve talking and listening to the patient's concerns as well as practical tasks such as collecting prescriptions, shopping and helping with meals.
This support, reassurance and encouragement can help relieve the stress and anxiety of going home and coping independently once again.
The Home from Hospital service is operated under contract to the Primary Care Trust and Socal Services. It is free to the patient.
